For the first time ever, Medicare started reimbursing for “hospital-at-home” in late 2020, leading hundreds of hospitals to sign up to deliver care in their patients’ homes. It’s part of a pandemic-fueled movement to push more care beyond the four walls of the hospital or clinic.  Join Tradeoffs for a virtual event that brings together administrators, clinicians, insurers, regulators, patients, and others with direct experience in at-home care (Is that you? See below for how you can join the conversation). We’ll share what we’ve learned about hospital at home from the past two years and what it tells us about the future of home-based care across the continuum — from hospital at home to long-term care to primary care.

More About the Featured Guests:

Bruce Leff, MD, is Professor of Medicine at the Johns Hopkins University Schools of Medicine. His research focuses on novel models of care delivery for older adults, multi-morbidity, risk prediction, and quality improvement, with an emphasis on home-based models of care, including Hospital at Home. He cares for patients in the acute, ambulatory and home settings.  He is past Chair of the Geriatric Medicine Board of the American Board of Internal Medicine and past-President of the American Academy of Home Care Physicians. He currently serves on the Board of Trustees of the American Board of Internal Medicine Foundation and the editorial board of the Annals of Internal Medicine.

Mona Siddiqui, MD, MPH, MSE is Senior Vice President for Clinical Operations for Humana’s Home Solutions business where she is leading the implementation of multiple initiatives to improve outcomes, drive efficiency and expand to new service lines.  Previously, Dr. Siddiqui led enterprise clinical strategy for Humana’s Insurance segment where she implemented approaches to reduce total cost of care and improve health outcomes across the Medicare Advantage population.  Prior to this, she has served in leadership roles at the Centers for Medicare and Medicaid Innovation, with the White House Social and Behavioral Sciences Team (“nudge” unit) during the Obama administration and at the Johns Hopkins Health System promoting value based care initiatives.

April J. Odom, MSN, APRN-FPA, CRRN, FNP-BC, a Family Nurse Practitioner with over 20 years of experience, practices as an Advanced Practice Registered Nurse (APRN-FPA) with Full Practice Authority in Illinois. In addition, she is a House Call Champion with the Home Centered Care Institute’s Illinois House Call Project. April is the CEO of Physicals Plus, a health and wellness clinic focusing on preventing chronic disease by offering vitamin therapy, medically supervised weight loss, and comprehensive care for families, including house calls to the homebound population.

Melanie Mercurio helped care for her husband David during his eight day Hospital at Home stay.  She is a retired Senior Administrative Assistant and spent 45 years in various medical fields including Radiology, Nuclear Medicine, Polysomnography, and Outpatient Psychiatry during her career of hospital employment. Melanie is pictured (left) with her husband David (right).
